Personalized Community Care (PCC) is a community-based, bed-based, intensive treatment program that promotes the stabilization and resolution of mental health and behavioural crises and improved functioning in children/youth up to the age of 19 years.

Personalized Community Care (PCC) is a specially designed service delivery model supports and treats the multifaceted complex characteristics and acute safety needs of children/youth within a highly staffed community care environment that incorporates a high level of supervision, structured therapeutic activities, and clinical oversight.

Within a developmentally and culturally responsive, clinically directed, trauma-informed therapeutic milieu, services provide, a total system of care that is tailored to a youth with severely maladjusted behaviour.

*All youth are referred by Children’s Services and have core protection status.
